Let's say you are on Orange. Phone 07973100450 and enter your phone number when prompted. Press 2, then 2. The system will ask you to enter you mobile number again for security. You need to take your phone into the shop that your sim card is with (Orange, Vodaphone, T-mobile) and they will give you the right code for absolutley free.

PUK Codes can only be obtained from your service provider. You can either call them or you can find the code in the papers that came with the SIM card.
