In my case a w755 model phone, if it just reboots and goes back to the bootloader usb init bluescreen no matter what, even if you take out the battery for a minute or try rebooting it with no battery and just the AC adapter. If nothing works, try tethering the phone to your computer and run 'motorola software update', it was able to connect, update/uncorruptify the software on the phone and now it works fine again.

In my case the above solution worked the first time. But, after about a month the same screen appeared again with the same symptoms. Retethering the phone did my work.

I think it is a hardware issue. I stuck it in the freezer for a while, didn't fix it. So then I carefully put it in my toaster oven on a piece of tin foil and let it heat the phone to about 200 degrees Fahrenheit, or about 2 minutes on "toast" (upper and lower elements).. The outside and inside screen turn black because they are too hot, but they will recover once they cool down. Take out the battery and put it back in. The phone will boot up properly now and will function fine, at least it has for me for about a month until the screen popped up again. I repeated the toaster oven procedure again and once again the phone is working fine. Very little permanent warpage on the phone.
