Twenty-Five questions, You need to get 18 correct in order to pass the permit test. & Dont listen to others who say you need to only study the signs or the fines. Study the entire book. And Good Luck : )
You can't take the permit test home but there is a driver's manual that you can take home or look up on the internet and study off of. After you study the manual you can take permit sample tests online to see what your score is and if you need to study more.

No you do not. Just go get the book from the MVD and study up, and if you know the content of the book just walk in there and ask to take your test. There is no requirement for driving school.
The best way to pass your permit test is to study. However if you find this difficult to do on your own a good way is to get a study partner who also needs to pass there test. Another way to help you study would be to have someone ask you questions and require you to answer them correctly, possibly making a game out of it.
