December 5, 1998, is when it became mandatory to dial the area code on all local calls to or from Dallas.
In 1996, the 214 area code for Dallas split, with downtown keeping 214 and the suburbs and outer parts of the city switching to 972. At that time, it became necessary to dial the area code for calls from 214 to 972 or vice-versa, but you could still dial just 7 digits for calls within either code. On 1998-12-05, the split was erased and turned into an overlay, requiring 10-digit dialing for all local calls.
Up until some time around 1990, you could dial 7 digits for any local call to or from Dallas, even if it was in a different area code (for example, 817 "metro" numbers in Fort Worth or Arlington). However, that arrangement meant that many prefixes had to be reserved, since there couldn't be both an 817-265 metro prefix and a 214-265 prefix. Requiring the area code for local calls into a neighboring area code allowed those reserved prefixes to be used.

Beginning in 2001, in preparation for the 878 overlay area code (which serves the same geographic territory as the 412 and 724 area codes combined), all local calls in area codes 412, 724, and 878 require 10-digit dialing.
Direct dialing just means that you dial the number yourself, without asking an operator to dial it for you.
The Denver area began mandatory 10-digit dialing of local calls in 1998, in preparation for the 303/720 area code overlay. All local calls within, between, into, or out from the 303 and 720 area codes must be dialed with 10 digits. Other local calls within area code 970 or within area code 719 may be dialable as either 7 or 10 digits.
There is not a good correlation between area codes and local calling areas in the United States. In some places, there are local calls to adjacent or nearby area codes. At the same time, a number in the same area code may be a toll call. Texas has something called "toll alerting." That means that if a number is a local call, you can dial it from a landline without dialing "one plus." If you are prompted to hang up and dial 1 to complete your call, then that is going to be considered a long distance call. The situation for the Dallas/Ft. Worth metropolitan area is complicated by the use of "Metro" numbers, which have a wider local calling area than non-Metro numbers in the same city. For example, a Ft. Worth Metro number is local to or from all of Dallas, but an ordinary Ft. Worth number to an ordinary Dallas number is a toll call. Most calls within area code 817 and overlay area code 682 are local calls. Some numbers in Dallas (214/972/469) and other nearby codes (254, 940, and 903/430) might also be local. Anything beyond that is definitely toll. The easiest way to find out if a number in North Texas is local or toll is to try dialing it on a regular landline with just the area code and number (e.g., 682-555-0123 instead of 1-682-555-0123). If the call goes through, it's local.
The dialing code +973 is for Bahrain. This country code is used when making international calls to Bahrain from abroad. When dialing, you would start with the international dialing prefix, followed by 973 and then the local number.
The international and local area dialing codes would all be the same, but your long distance company of choice would have to provide you with their direct or operator assisted dialing protocols.

Dialing code +39 is the international telephone code for Italy. It is used when making international calls to Italy from other countries. When dialing from abroad, callers must first enter their country's exit code, followed by 39, and then the local number.
