Many different people. Often it is the person who requests the category, but sometimes it could be someone who does the research and realizes the spelling needs to be corrected, or a category should encompass something else, or any of a number of other things. Or, someone sees that a category name needs to be changed, and asks to have it changed.
As of March 16, 2009, there are only 4 questions on WikiAnswers about Girlfriend magazine. If and when there are about 30 questions, WikiAnswers can open up a category for this specific magazine.
It may take a few days for your category request to be approved and added by a Community Assistant onto WikiAnswers. Community Assistants are very busy on the whole of WikiAnswers, but will try to sort out the category request ASAP. For the most chance of getting a category added remember to include in the email where it would be placed and if there are sufficient questions to fill it (we look for 50+ questions in the category before making it, however exceptions can be made).
Usually, the system chooses up to three suitable categories - based on the wording of the question.

There are currently not enough questions on this band to warrant opening up a category of its own. If and when we do have sufficient questions on Aerosmith, however, we would be happy to open such a subcategory.

Well, it is mainly up to you, Don't forget, a Category Supervisor looks after certain categories, whereas a floating Supervisor maintains every category, just 'Floating'. Hence the name. So it depends whether you prefer just one category or not.
It makes the names easier to remember.
There is no easy way to merge categories at this time. If you think that two categories should be merged, you can write to WikiAnswers @ Answers.com (without spaces) and request it.Another possibility is that you have run into a "mirror" category, where the same category with the exact same questions shows up in two different places within our category tree. We do this sometimes to make them easier to find if they are popular in different areas.
