Probably an issue with the main line. Tree roots possible or collapsing. One thing you might try if it is just buildup in the line. Get a 5 gallon bucket full of water and pour it in the toilet as fast as it will take it. Enough so that the level in the bowl comes up to the underneath side of the rim. This may open up the line for a while.

What would cause a toilet to completely flush every other time and it always has to be flushed twice?

This could indicate a problem with the flushing mechanism. It's possible that the flapper valve is not sealing properly on every flush, leading to incomplete flushing. Additionally, there may be a partial clog in the toilet's drain pipe, requiring a second flush to fully clear the bowl. A plumber can inspect and repair the toilet to resolve these issues.
