If this is a shower handle, this is a common mistake installers make. Just remove the handle, and rotate the pin that the handle connects to by 180 degrees. This will reverse the mixing cartridge. If this is a sink, then your water supply lines are reversed. Just shut the water off, and switch the lines supplying water under the sink.

I think the only reasonable answer can be that the cold and hot water inlet/outlets are reversed. your hot water line is attached, at the heater to the cold water inlet or your hot water outlet is too short inside of the tank
You will get warm instead of hot water. There is a tube inside the tank on the cold side that goes to the bottom of the tank and the hot comes directly from the top. Reverse them and you are going to get the cold water that settles to the bottom of the tank. It will not hurt the tank or anything, it just makes for poor hot water.
