Caleb, son of Kansas state Rep. Scott Schwab, was on Verruckt, the world's tallest water slide.

The combined minimum weight of the riders is a required 400 pounds, which the three riders did not meet. This was despite the fact that there was a scale to weigh riders as they got on the ride. At the top of one of the hills, the raft went airborne.

Caleb's velcro straps failed, and he launched into a metal hoop at 65 mph. It was at this point that he was decapitated. His head hit the other women in the raft, breaking the jaw of one woman, and causing an eye injury to another.

His brother witnessed the entire incident. It has been reported by other park guests that they informed the ride operators that the velcro straps had failed for them during the ride, but that the operator blew them off. One guest said that the operator told him that the straps were just there for looks anyway.

There is also video from other guests showing the straps coming loose from another rider, earlier that day.
