A locomotive entered the wrong tunnel and plunged down a mine shaft, striking the safety detaching hooks mounted on top of the "cage" (elevator for a mine), which are intended as safety devices designed to release the hoist ropes and prevent the cage from falling back down into shaft in the event of an "overwind" condition.

The safety detaching hooks were subsequently opened as a result of the impact, sending a 2-level cage containing over 100 miners plummeting 400 m to the bottom of the mine shaft, with the locomotive landing on top of it.
