In the game, Pokemon evolution stages are known as basic, stage 1, and stage 2.
Not one Pokemon can have 4 stages of evolution. I've never seen one either. But, it could probably be possible to use a hack, such as GameShark. I don't know how to use it, but it is possible. Again, no Pokemon has 4 stages of evolution unless you hack your game. The highest number of evolutionds I've seen is 2.

You can't. The name rater won't change names for Pokemon that someone else named... only for Pokemon that you named yourself. This is also true for Pokemon that you traded from another game... any other game, even if it was you playing it, the game can't tell, and so the names are permanent.

If there is something like, say, "*****," it means "This Pokemon and its evolution can only be found by using Surf." If there is "****," it means "This Pokemon and its evolution can only be found by fishing. Pikachu, Oddish***, Doduo***, Natu***, Wobbuffet, Girafirig, Psyduck*****, Rhyhorn, Pinsir, Goldeen****, Heracross, Phanpy, Geodude. I did not include any Emerald-only Pokemon. These are all Pokemon that appear in all three games.
