It really depends on what the questionnaire is meant to do. If the aim is to discover how many people are aware of local visual and performing arts venues, the questions would all relate to this basic theme. For example, Do you attend art galleries, plays, ballet, or symphony events? When was the last time you attended a performing arts venue? Do you know of local performances or art shows? and so on. Determine the aim of the questionnaire and the questions will come from that.
