You will have to collect a bank pay-in slip and another form from the reception. 2. Remit 200 rupees in favor of KSNC at the central bank(which is near to the KSNC office) 3. Fill the form with the reg no and the hospital address of the country in which you need this certificate for. 4. Attach the copy of your qualification certificate and chalan along with the form and submit the same at the officer desk in first floor (on the right hand side) Later they would fax the good standing certificate to the hospital via speed post. (candidate is not necessarily to be present while submitting the form)
