Midland Bank changed their name to HSBC many years ago. Contact them with as much info as you have available; full name, last address, security details etc., and they should be able to trace the account for you but you will have to prove that you were the account holder.
I don't think that there was ever an LMS bank but LMS Railway is now known as London Midland.
