Based on historical records of significant tornadoes in the Carolinas, the worst outbreaks to impact those states would be:

March 28, 1984

A series of tornadoes, mostly relating to two tornado families tore across the Carolinas. Several over these were of F3 to F4 strength. In all, 57 people were killed, all in the Carolinas.

April 29-30, 1924

A major outbreak occurred, starting over Oklahoma and moving east. In all, 114 people were killed, including at least 72 people in the Carolinas. A violent tornado family killed 53 people across South Carolina.

February 19-20, 1884

Several strong to violent tornadoes struck the South. At least 167 people were killed in all, most of them in Georgia. Due to large uncertainties in the death toll, this event is sometimes referred to as the Enigma Outbreak. The were at least 56 deaths in the Carolinas, including 23 in an F4 tornado that hit Pee Dee and Philadelphia, North Carolina.

Yes, in fact some places have been hit twice in the same day. On April 3, 1974 the towns of Tanner, Capshaw , and Harvest, Alabama were first hit by an F5 tornado, followed by an F4 (listed by some as F5) tornado barely 30 minutes later. In some places it was impossible to tell which tornado damage what.
