  • If your passport was issued less than 15 years ago, and you were at least 16 years old when it was issued, then you can do the DS-82 mail-in form. You would need to fill out the form, attach a new photograph, and then send it in along with the old passport and a check for the passport fees.   • In all other cases, or if you simply don't want to do the DS-82, you do a DS-11 form. This involves bringing it to an "acceptance facility" (normally a local library or post office), swearing an oath, and so on. You would need to send in the old passport along with the DS-11, and pay a little extra to the acceptance agent for their services.

These forms and their instructions, as well as a directory of acceptance facilities, can be found on the US State Department's passport website, travel.state.gov. You will normally get back your old passport, which will have been canceled, along with your new one.

If you're not an American, then you would have to refer to your own country's passport issuance agency.
