I've never had to take one in three combat tours, but I've seen some units which did administer them. Typically, these would be units which never left the FOB. However, in combat, there are more pressing matters than one's PT score.
On a regular army base (i.e. not in a combat zone) combat personnel do not carry weapons on their person at all times. However, if the base is in a combat zone, all personnel are required to carry weapons. These arms can be anything from a personal weapon (pistol) to a combat ready device or assault rifle. It depends solely on the weaponry that is assigned to the combat personnel.
Those in a combat zone usually slept, ate, and read or wrote letters. Those not in a zone of combat were usually training and getting ready to move into a combat zone. No matter which army they served soldiers had the same desires, rest, food, and family.

Where are you going on leave from? If you're taking your block leave from a non-combat zone, you pay your own transportation costs. If you're taking leave from a combat zone, the military will get you stateside, although you may have to pay your own costs from your port of entry to your final destination.
