Medicaid is a heath delivery system that is essentially a partnership between the federal government and the states. The federal government provides the funding from general revenue obtained by taxation, and the states administer it and set the income criteria for entitlement. Entitlement demands low income and is based upon individual income for an individual recipient, or family income at a certain level depending upon the size of the family. The idea of a supplement to Medicaid is an anomaly. The health care provider that accepts Medicaid agrees to accept the Medicaid reimbursement rate for services rendered. If the patient does not wish to use a Medicaid provider, he/she is free to seek another provider. It is always important to remember that medical care is like any other commodity in the sense that fees can be negotiated with the provider, especially in return for cash. Therefore, if, for example, surgery is planned, you can shop qualified care providers and facilities and negotiate fees. Again, cash speaks loudly. Naturally, a different analysis may apply if an emergency is involved. Nonetheless, money can be accumulated in a savings account designated for medical usage, in a tax-advantaged medical savings account, or, by purchasing a "mini-med" policy. This bears a resemblance to a major medical policy but is intended for use in catastrophic circumstances. There is a large deductible, meaning that you have to pay, out of pocket, the deductible before the indemnity of the insurance policy kicks in. But even then, remember that the fees of the provider are negotiable, so use that bargaining power.
