Make sure that the songs that were downloaded onto your iPod are in MP3 format. Songs downloaded from iTunes are not in MP3 format. Even though you can hear the songs on your iPod when you connect headphones to it, the iPod port in your car might not recogonize the songs. The songs on your son's iPod might be in MP3 and that is why you could hear it.
Yes you do have to pay for music on your iPod touch. Some songs are $0.69 to $1.29 but they can also be $0.99. You can buy them from your computer or from your iTunes app on the iPod. You can also put music that you already have onto iTunes and then transfer it to your iPod.

To get your songs onto your new iPod Shuffle, first connect it to your computer and open iTunes (or Finder on macOS Catalina and later). If your purchased songs are in your iTunes library, they will automatically sync if you set your iPod to sync with your library. For your own songs, you can drag and drop them into your iTunes library and then sync them to your iPod Shuffle. Make sure to create a playlist if you want to organize your songs before syncing.
Spotify is a streaming service they do not provide songs for download. In some countries they provide a link so you can buy the song and use that on your iPod. If you have a Premium account with Spotify you can store songs to play on an iPhone or iPod Touch, and some other mobile phones, when not connected to Spotify.
