AnswerIf one of the secondaries should actually be tertiary then eventually it will come after the $$$. If the two secondaries are supposed to ignore one another (they do not coordinate with anything other than Medicare) then the overpayments should go to the policyholder.

Retiree health care coverage and Medicare?

Question isn't clear, but ordinarily Medicare is the primary payor and your retiree coverage is secondary.

Can you buy Medicare supplemental insurance if you have Medicare carve out coverage?

Please explain your question more thoroughly if my answer does not suffice. I am unsure of what you mean by Medicare Carve Out Coverage. You can buy a Medicare Supplement at any time once you have received your Medicare Part A and Part B. If you do not enroll within 6 months of your Part B effective date you would be subject to underwriting. You can not join a Medicare Supplement if you already have a Medicare Advantage Plan as this is not allowed by Centers for Medicare. You would be required to drop your Medicare Advantage Plan prior to the Medicare Supplement effective date. If you had coverage through an employer, you would not need Medicare Supplement coverage as your employer coverage would be primary and then Medicare would be secondary for your out-of-pocket costs covered by Medicare.

Does medicare part a pay the difference after Blue Cross which is primary carrier paid most of the claim for skilled nursing facility I have a 290 balance due the nursing facility?

Medicare does offer coverage for skilled nursing facilties. In order to find out if Medicare will pay as your secondary, the provider needs to submit it to Medicare. This statement is from the Meidcare.gov website: Medicare providers must submit claims (bills) to Medicare for you, whether Medicare is your primary or secondary insurer. For Medicare to process a claim as a secondary payer, the provider must give your primary insurance information to Medicare. You may also consider calling 1-800-Medicare for information about secondary coverage. If you do, remember from Nov 15th to Dec 31st is a busy time for Medicare so it may be difficult to reach them. One more hint to save some frustration: If BlueCross BlueShield has already paid the amount they were supposed to pay, calling them won't really help you because their job is done. Now the remaining bill is between the provider and Medicare.

You want blue cross to be primary and medicare to secondary can you do that?

No. There is a federal law that requires Medicare is a secondary payer. You can find more information on the CMS website under "Medicare Secondary Payer and You."
