That is a term that refers to the ribbons worn by soldiers on their uniform above the LEFT pockect. These ribbons represent various Medals earned by the soldier. Some Medals are earned for bravery in combat, some are for serving in campaigns or Wars and some are issued in recognition. So, the term "combat" may refer just to the medals earned in war but not necessarily. Another slang word for the variety of colorful ribbons is "salad".

Yes, but the only ribbons you can wear are the ribbons that does not have a corresponding Medal i.e Combat Action Ribbon, Seaservice Deployment Ribbon. You can wear up to 7 medals in one row but they have to be mounted a special way. Ribbons are worn on the right hand side and follow the 1/8th on an inch spacing from the pocket.

According to the current AR 670-1, (Feb 3rd, 2005 w/ Summary of Change) on pg 298, you wear the CAB, (Combat Action Badge) 1/4 inch above your ribbons, centered, or if the lapel covers your badge, you wear the CAB so the left edge is aligned to the left edge of the ribbons.
