OSHA is a federal agency and has juristiction over county health departments.
State inspections for health and safety are done by the county health department. Contact your local department for a copy of the restaurant inspection. Many health departments post these results online.

It varies by state and county. I'm in California and the costs are roughly as follows: You must submit plans ($400-1600 in our county), then purchase a health permit after approval ($200-800 in our county), then have health inspections 1,2, or 3 times a year (included in the cost of permit). Reinspections after failures or other time spent by the health department outside of the permit schedule is charged at a predetermined rate (currently $145/hr in our county)
