The unofficial policy began with George Washington, the first US president (1789-1897). It was informally recognized until 1940, when Franklin Delano Roosevelt ran for (and won) a third term during World War II. His popularity was sufficient to allow this, and he was then elected to a 4th term in 1944, with the war nearly at an end. He only served three months of his 4th term, dying in April 1945.
The continued concern over setting a precedent resulted in the passage of the 22nd Amendment, ratified in 1946, which limits a US president to two full elected terms.
(He may also serve up to two years as a successor to another President -- if he serves more than two years, he may only have one additional four-year term.)

No. The Constitution of the United States only allows for two consecutive terms of any one administration. That does not mean that a President who has served two consecutive terms cannot nominate and run for Presidency in the future.

No, that is not true. Only US citizens who were born in the US qualify to run for the Presidency. Naturalized citizens born in another country do not qualify.
Barack Obama can run for two terms, back to back. But after that he has to wait four years before he runs for another two terms. And he can do that as long as he wants
The first US President was George Washington. He served two terms and his presidency ended in 1797. His 2 term limit, thus 8 years of presidency became a benchmark for all future US presidents until President Franklin Delano Roosevelt ran 4 times.George Washington died in 1799, thus his retirement was but only 2 years.
