The Chief of Staff is a position appointed by the President. If the elected President and Vice-President are unable to serve, the Speaker of the House of Representatives becomes President and selects a Chief of Staff.
It depends on the legislation of the country concerned, but generally the Vice President, if there is one would take over the duties.

The President can resume their duties once they inform Congress that they are able to do so.
