There are several possibilities:

  • The requirement may be written poorly. If you are told to feed your friends goldfish and he says "Remember you can't feed goldfish too much" does he mean they die if fed to much or they can eat however much you give them.
  • The requirement may be written assuming you know information that you don't. An example " Adjust the main dial within the parameters of ISO 1372(b)"
  • The requirement may be contradictory "Go ahead and stop."
  • The instruction may be described in a manner that is not not an absolute or quantifiable. Example: "... add enough detergent to the washing machine" instead of "... add a half cup of detergent to the washing machine"
  • The instructions may be buried in a lot of irrelevant details
  • The instructions may be out of date
