Yes, the option is given once, when you first start up a copy of MW2. However, i would only advise skipping the level if a young child will be viewing it, or if you are greatly offended by the death of civilians. I feel that it is a level that should be experienced. It is handled very tastefully and adds real impact to the campaign. Not to mention the story as a whole doesnt make sense without viewing the events that take place in the "No Russian" level.
NOTE: The player is NOT required to kill unarmed civilians in "No Russian". The player can just wait behind the other members of the Russian Ultranationalist group and let them do the killing. The player only really has to participate by defending himself when the armed Russian Anti-Terrorism team comes at the end
