I had that same question a while ago. But then I clicked a few things and found out! If you've had several children go to college, only one will come back. That one person will come back once a parent has died. If there are any kids left that the parents had, they will be vanished from the screen. You click the "Start a new generation" Button when you are at the family tree. You get there by clicking "Family"

There is no doctor. You have to get your people checked for $175. It's near all the medicines (stethoscope). Easy way to save money though is pick them up and drop them back down. They will say what there problem is and you can just buy medicine for whatever they say. :)
