Sora ends up in Traverse Town after his home-world, "Destiny Islands" is overtaken and (temporarily) destroyed by darkness. "Traverse Town" is a world that holds former residents of several worlds that have been destroyed by darkness, so it's almost natural that Sora ends up there.

Sora and Roxas met when Donald, Goofy, and Sora were walking before a giant building in Twilight Town. Suddenly, Heartless appeared and distracted Donald and Goofy. Meanwhile, Sora suddenly found himself in the Station of Serenity in The World the Never Was, where you have to choose the Sword, the Staff, or the Shield (Strength, Magic, or Defense). Then, Sora looks at Roxas in his Organization XIII suit, and they fight a brief duel. After that, Roxas turns to Sora and says, "Hmph! You're a good other," and disappears. Then, Sora returns to Twilight Town.

Two conceivable ways are that DiZ, after being unsuccessfully attacked by Roxas, forced Roxas out of the virtual town and reunited him with the real Sora, or that the virtual Twilight Town has enough of a spiritual connection with the real Twilight Town (implied as the real Pence seems to recall Sora in some way, which should've been impossible) to allow Roxas to reunite with Sora through dataspace. The latter seems to be the one people just follow to save themselves the thinking, though I agree that it's an anomaly missed by the creators; we can only speculate to decide what actually happened.

No he does not. This is obvious in the first scene we see Sora where he is confused about the entry in the journal to "thank Namine." None of the characters understand why they need to think her. Now, some may speculate that Roxas being a part of Sora would remember her because of his interaction with Namine during the prologue. However, this can be contradicted by the fact that the Twilight Town in the prologue is not the real Twilight Town. It is a computer re-creation of the town. All of the events that occur in the town really did happen, but it was used as a way to show Sora's memories being rebuilt from another side. Because, if Sora lost his memories, then Roxas would have lost his because they are two sides to one coin. The prologue was an interesting way to show the rebuilding of Sora's memories while giving us a new character to connect with and grow to appreciate.
