To build a new city you need to have a title high enough to entitle you to an additional city. Each title upgrade allows you one more city. So at Knight you can have two cities, Baronet 3 cities, etc.
First thing you do is conquer an empty flat, any flat will do. Easiest one to conquer is a low level flat. Then, if your title allows, you just click on Build City and it will automatically build. Takes maybe 10 minutes. NOTE: You must have the following to Build City: 250 workers, and 10,000 of each resource.
If you want to build an NPC, just abandon the new city after you build it. Go to TownHall, select City, select Abandon. The abandoned city automatically becomes an NPC at the level of the original empty flat.
