You can only change the gender of a child (or, at least influence it) before the child is born, during the first tremester. This you can do by eating watermelons (to get a girl) or apples (to get a boy). The more of the particular fruit your pregnant sim eats the more likely she is to have the gender. However once your sim is noticably pregnant this will have no effect.

Once the child is born you can change it using cheats, although this doesn't change it properly and you may end up with some odd mistakes during gameplay such as your child being recognised as the opposite gender. This you can do by opening the cheat panel (ctrl+shift+C, or ctrl+shift+alt+C on a Mac) and typing 'testingcheatsenabled true'. This is the most powerful cheat on Sims 3. Shift click on the child and choose 'Edit Sim in CAS'. This takes you to the Create A Household screen where you can edit the child as though you were first creating them. Change their gender and click the tick at the bottom of the screen to finish.
