HeartGold, SoulSilver, Diamond, Pearl and Platinum are fourth generation games. They're ineligible for transfer/transporter use.
You must have unlocked the transfer lab on route 15, and you must have at least two Nintendo DS systems. (DS "phat", DS Lite, DSi, DSi XL, 2DS, 3DS, 3DS XL, New 3DS or New 3DS XL) Enter the transfer lab and talk to the guy at the desk and follow the onscreen instructions. You'll have to play a minigame to transfer your Pokémon, after which you can transfer them to the fifth generation. (If you lose the minigame, don't worry -- the transfer is aborted and your Pokémon will be returned to their boxes in the fourth gen game)
You can only transfer six Pokémon at a time. When you've completed the transfer(s),, purchase Pokémon Bank and launch Pokémon Transporter (bundled with the purchase) with Black/Black 2/White/White 2 inserted into your 3DS. Follow the on-screen instructions and repeat as-needed.
