Go to the Gerudo Fortress. Sink to the bottom of the water pit in the east room and defeat the enemies. Then let things settle for about 10 seconds. afterward look around the room (still underwater) with the hookshot. Then look behind you. Ben Drowned will pop in your face with a bluish glow and fade away as demonic music plays.
REMINDER: You must have all hearts and Gear except for the Ice Arrow to perform. I also believe that this is where Ben Drowned actually drowned before Majora's Mask! Easter Egg :)!
