Clean the disk of any dirt or smudges, make sure its not scratched.
I'm assuming that you have the GTA 4 disk inserted in, but you want to play one of the DLCs. In which case, simply insert your Episodes of Liberty City disk into the console and load from there
yes i know the best solution is you have to do a fresh gta 4 installation. it is the problem with the save games

If you are playing GTA IV on a games disk, then it could be that the disk is at fault. The first two GTA IV games disks I had were faulty like this, but I changed them each time, and the last disk I had is absolutely fine. If you can play other games on your console and they are OK, then it would point to this. You could also ask a friend who has the same game to loan you their disk and then see how it plays on your console. If their copy is fine, then you would seem to have a disk problem. Hope you get it sorted out OK.
ive had this happen to many gta game disc for some reason this is common specialy in gta vc sadly there realy isn't any way to fix it that i know of just buy a new disc

First, to play GTA IV free you need a blank disk and a torrent application. I suggest you use FinalTorrent. Search the download section for GTA IV applications and download. It will ask you to burn the torrent. You NEED the disk for it to work. Burn the torrent to the disk and that's for GTA. Now, go to lcpdfr.com and download something like RC2 LCPDfr 0.95 update . After its done downloading. if I'm right, go to campaign and press Alt-2 or go to a police station and start patrolling. Have fun.
If you are playing with a games disk in your console, and the game gets "stuck" in the same place at a certain point every time, it is possibly because your disk is damaged. I am no expert, but something similar happened to me. I borrowed a friend's disk of the same game, and it was fine, so I knew it was the disk I had. I bought another one and it was OK after that. I did try to give the disk a good clean, because sometimes that works, but it didn't in this case. I was glad it was the disk and not the games console that had a problem! Perhaps you can borrow someone's disk just to test? I hope you get it sorted out OK.
