How do you get out of the sewer in Zelda twilight princess?

You have to pull the various chains to raise and lower the water level when needed while avoiding the little black monsters until you get to the room with the circular staircase and then make your way to the top with the help of Midna. I recommend you use the walkthrough at ign.com

To get out you need to get to the highest castle and find Zelda if you are stuck in the jail attack the haystack and dig out in that specific place
