After vacuuming all the ghosts in the Tea Room, look for one of those switches that let you walk on the ceiling when you step on them (I think it's in the bottom right part of the room). There will be another switch on the ceiling, and when you step on it, it brings you down on top of the table where the treasure chest containing the Ice Medal is.
