To unlock all three zombie maps on Black Ops, on he main menu, repeatedly tap R1 and L1 to get out of your chair. Then go to the terminal to your left. Once there, enter the code "3ARC UNLOCK" to unlock the zombie map "Five" and "Dead Ops Arcade". I hope this helped!

the three zombie maps are kino der toten, five and dead ops arcade (and also ascension if you have the first strike map pack.) in order to get these maps you must do certain things.
to get five you must complete the campaign of the game.
to get dead ops arcade look down at your hands and you must have a classic controller and pull ZL and ZR in order to get up from the chair and find the computer. when at the computer type in DOA and you will get the dead ops arcade game.
to get ascension buy the first strike map pack
alternatively you can get all of the maps except for ascencion by going to the computer once again and entering the code 3ARC UNLOCK.
