Go to the upper left and back up the throat, and float up to the mouth (your shield is by Joe's gold tooth). Then you can dive through the grape juice and stomach acid in the stomach to reach the intestines, where you recover the scalpel.
How do you leave from the heart on virus hunter island?
When you are first in the heart, battling the virus, you have to double back out the way you came in, because going forward takes you prematurely to the lungs (you have no gravity core). Leave the heart at the upper left and go to the arm, where you will use the coagulator that you have just recovered. When you have completed the arm and the brain, return to the heart and travel through to the upper right, where you exit to the lungs (the final virus boss).
