Once you get the fountain, interact with it and you must have a regular axe with you and you can throw it in there. Serena the Goddess would ask you if threw the axe and she'll ask more questions and she'll either give you: Silver axe, Golden axe, or just a regular axe. Or she could just take and keep the regular axe.
