there are 3 forms of sasuke on naruto ultimate ninja storm 2, his kirin form and chidori true spear form are unlocked as you play the storyline, to unlock sasuke taka (which is sasuke wearing an akatsuki cloak) you need to clear the fragment story which you can play by completing the main story then talking to tsunade in the hokage building

Sasuke doesn't have a demon form exactly. It's the second stage of his Curse Mark, I think you're thinking of. Um, at the Final Valley or something in Pre-Shippuden, Naruto goes Nine-tail on Sasuke, and Sasuke goes into his second stage.
