The only reason you would be locked out, is if there are Super Mutants roaming around; usually this only happens when you first go to GNR and meet up with Sarah Lyons and the Lyons Pride from the Brotherhood of steel. Steel Be With You!
you have to sacrafice Sarah from the brotherhood(you have to do the main story line) to go into purifier and you will get knocked out it will look like the game is ending but its not you will wake up two weeks later and you will be talking to Elder Lyons

The brotherhood outcast will come after you... but don't worry it's not much of a problem as along as you have some laser rifles or assault rifles... WORD of advice. If you are going to kill the outcast... make sure you raid fort independence to get some powerful weapons like the gastling laser.. or PLASMA rifles. ( this only applies the fallout 3 in dc and not fallout 3 new Vegas)
