I assume you mean the god book. I couldn't find information on whether it stays with a gravestone, but it would not be a major catastrophe if it doesn't. From runescape.wikia: "If a player loses their God book, they can go back to Jossik who will have found it 'on the shore' and get it back for free, complete with pages that have been added."

When you die right now in Runescape you do get a grave.
Runescape is a computer game, an inanimate production, i.e. not a living thing, so it can't die.
If it didn't die just log out and back in, should be back with you. If it does not reappear then it died.
Because runescape has to suitable for younger children, so there can be no vivid or detailed death animation
nope but if you can get to your grave you can pick you stuff back up this is only if it is still there it will collapse in a few minutes unless somebody has blessed it or fixed it
