The hidden blade as shown within the game is a fantasy weapon. While it may be backed by some mechanical work, designing one with the identical use and look may be impossible.
Official concept art of Altair's hidden blade show a mechanism similar to a scissor lift, activated by the tug of pinky ring.
Ezio's blade is a different style and most likely operates on a system of springs and locks.
You get the hidden blade after you talk to your father Giovanni, and after you talk to Leonardo. Leonardo fixes it for you using the Codex page you received from an earlier memory sequence. You use X to use it when you are not locked on to a target.
If you mean in real life, yes it is possible. There are two possible ways it could work:
A) it has a lock, and when you flick your wrist, blade comes out.
B) it has a grip that comes to your hand, you squeeze, blade comes out.
"A" is the way it works in Assassin's Creed.
