There is no "code" to unlock Five. In order to unlock five, you must first complete the Campaign on any difficulty. However, if you break away from the chair at the main menu (Look Down and press both triggers at the same time) and go to the Computer at the back of the room and type in "DOA", (which by the way stands for "Dead on Arrival") you will unlock the Game Type "Dead Ops Arcade". Well it depends on what version of the game you have. It is NOT on Wii and I don't think it is on DS either. To unlock it in PS3, Xbox 360, and PC you can:

1. Complete the Campaign

2. Break out of the seat and go to the computer where you can enter codes and enter 3ARC UNLOCK.

The map is unlocked after finishing the campaign on any difficulty, or typing "3ARC UNLOCK" on the computer via the main menu. This map is set in the Pentagon. go behin your chair and you should see a coputer log onto it and enter the code 3ARC UNLOCK get back on the chair look in Zombies and you should have Five and Doa(Dead Ops Arcade)
