It should not. it would be like becoming a warewolf. The people fear you but a lv4 vampire does not get attacked. the vampire lord should not effect gameplay except that removing dawnguard may cause problems. the vampire lord is weakened by the sun but you can still get the well rested bonus.

If you have the Dawnguard DLC, then you can just go along with the Dawnguard Quest-line and choose to become the updated form of Vampire. In the "vanilla game" (game without dlc), you must be given the "Sanguinare Vampiris" disease by a Vampire. This is possible by being struck by a vampire and is preventable if you drink a cure poison potion. Vampires won't often be found in cities unless you have Dawnguard DLC, otherwise... go to a vampire dungeon like Movarth's Lair (outside Morthal).
