If you are still stuck, try going to Home, then the Store, and get the Haunted House card, which is the only way initially to leave Virus Hunter Island. Several glitches were the result of programming errors that locked up browsers during the first month of the island's appearance.
Go back to the stomach when you leave the hand. But the acid is too strong without your shield. Go to the upper left and back up the throat, and float up to the mouth (your shield is by Joe's gold tooth). Then you can dive through the grape juice and stomach acid in the stomach to reach the intestines, where you recover the scalpel.
