Kingdom Hearts coded (first released on November 8, 2008) is the 5th Kingdom Hearts game, but it's only playable on cell phones in Japan. It was released in 8 episodes as of January 2010.
Now, it has a remake version titled Kingdom Hearts Re:coded, which was released in Japan on October 7, 2010, in the US on January 11, 2011, and in the UK on January 14, 2011. Re:coded will only be playable on Nintendo DS.
The game sets after after Kingdom Hearts 2, where Jiminy Cricket finds something interesting in his notebook and consults Mickey. You only get to play as Data Sora (a digital version of Sora) in this game and you get no partners.

Yes. At E3, two Kingdom Hearts titles were announced; Kingdom Hearts 3D and Kingdom Hearts RE: Coded. Not much information is available regarding Kingdom Hearts 3D, but it will be for the Nintendo 3DS and will utilize the 3DS's new 3D feaures. Kingdom Hearts RE: Coded is just Kingdom Hearts Coded, remade for the Nintendo DS.
