lol that's the nice sounding campaign rating i would say that the online rating is

violence, gore, MASSIVE adult language, and a lot of drug references

All FPS are M rated because you can not shoot someone without showing intense violence and blood or maybe even gore unless you do not make it realistic.. That earns the M rating and once it is rated M additional items like strong language do not effect the rating
