To get the eye of truth you have to go to Kakariko Village as an adult after you beat the water temple. When you arrive the town will be on fire and a monster has escaped. Shiek will be attacked and then disappear. go to the windmill and talk to the guy with the funky instrument. He will play you a song for the ocarina. Now after that you go to the Temple of Time and put the Master Sword back on its pedestal to turn into Young Link. Then go to Kakariko Village and play that song the guy taught you in the windmill. The well will dry up so go to the bottom and battle your way through; you will find the eye of truth in there.
The eye of truth is found in kakariko village as young link. It's in the well in the middle of town, but you need adult link's help to get there. You need to go into the windmill as an adult and learn the song of storms from the music guy, talk to him then pull out your ocarina in front of him. Learn the song, then go back in time and play the song in the windmill. if that doesn't work, do it in front of the well.
The water will be drained out of the well, allowing to to go in. complete the dungeon, and you get the eye of truth.
I am simply going to assume you have already completed the quests as child Link (collecting the three spiritual stones and opening the Door of Time). I am also assuming you have already taken the Master Sword, and completed your first quest.
Now, you need to go to Kakoriko village, and to the windmill. Go inside, and go up to the crazy man playing the Accordion or whatever it is. He will teach you The Song of Storms,cursing the kid that played it seven years before and ruined everything.
Return to the Temple of Time, and replace the Master Sword. This will send you back in time seven years, returning you to your child form. Go back to Kakoriko, and go into the windmill. Play The Song of Storms to the man. Ha ha ha! Little did he know HE was the one who taught the song to the kid who would ruin his life with it!!! Sorry, I just think that's brilliant.
Anyways, playing the song will lower the water in the well. Go outside, climb down the well, and crawl through the little hole. You are now ready to go through this mini-dungeon and find the Eye of Truth!

Get the Lens of TruthAfter learning the Song of Storms, play the Prelude of Light to return to the Temple of Time. Stand next to the pedestal and drop the Master Sword in the pedestal to become a child again. Return to Kakariko Village and enter the Windmill, then play the Song of Storms to lower the water in the well. Climb down into the well and enter the small hole in the next room. Climb down the ladder to enter the Bottom of the Well.
After you enter the bottom of the well, go forward to the far wall and WalkThrough it to the right of the skeleton. Go around the water and WalkThrough the middle of the far wall. Continue forward, around the crossed wooden beams, and through the middle of the far wall again. Stand on The Triforce symbol and play Zelda's Lullaby to lower the water. Turn around and go back through the middle of the next two walls. Drop into the pit and enter the small opening. Go forward, climb up the vines and enter the door to find an enemy named Dead Hand. Defeat Dead Hand then open the big treasure chest which appears for the Lens of Truth.
