Recently an update was made where you can change your display name. When you do this a symbol appears to the left side of their new name, in the shape of two heads with an arrow. You can change your username through the account management section on the Runescape main page.
Otherwise, if it is an asterisk (*) next to their name, it means the account with that display name has not logged in within the past 6 months.
