When Kingdom Hearts II came out in Japan, it took several months to be released in other regions. When the game was released in other regions, however, multiple additions were added to the game. Smoother graphics, added bosses, and much more were implemented to compensate the time-gap between Japan.
In order to 'make things even' for Japan, the additions were then implemented into the Final Mix version of the game, but also includes even more special features such as newer bosses, new worlds, and new weapons. This is the final edit, hence the name "Final Mix."

The Japanese haven´t released it in America, nor other countries. The Final Mix series of Kingdom Hearts are ONLY for Japan and Australia, not America, so, I guess the only way you can get this item is buying ONLINE.

The 'Final Mix' games (Kingdom Hearts Final Mix and Kingdom Hearts II Final Mix+) are Japan-only, but have an option for English subtitles. The only all-Japanese Kingdom Hearts title would be Kingdom Hearts Coded for the mobile phone. This game was released only in Japan, and isn't scheduled for release in America or any other PAL region.

Limit Form is only available in Kingdom Hearts II Final Mix+, which is only available in Japan. In Kingdom Hearts II Final Mix+, Limit Form is obtained at the same time you get the Oathkeeper Keyblade in Twilight Town (once Sora finds out about Kairi).
