In order to get the 3rd crystal, you must go to Skull Woods. This is the woods where you got the mastersword in the light world. All's you have to do is go through the temple and beat it. Make sure that you get the Fire Rod. You are going to need it for the boss. Other wises than that, the rest you should figure out on your own.
To free the fourth Maiden in A Link to the Past, you must defeat Blind the Thief, the boss of Thieves' Town, the fourth dungeon in the Dark World. Thieves' Town is located beneath Village of Outcasts and can be accessed by pulling on the trident "held" by the gargoyle statue in the middle of town and then using the stairs that are revealed.
